The Summit is sponsored by the Tennessee Pediatric Society Foundation (TPSF), the Tennessee Chapter of the American Academy of Pediatrics (TNAAP) and the Tennessee Department of Health (TDOH).

Learning objectives for the Tennessee Transition Summit will include:

• Knowledge of the evidence base and recommendations behind the AAP, AAFP, and ACP 2002 and 2011 consensus statements on Adolescent Transition and Transition on Adolescents with Special Healthcare Needs.
• Familiarity with practical operational guidance on transition implementation in the medical home.
• Familiarity with Tennessee specific transition resources pertaining to guardianship, insurance, SSI, and resources for independent living for transitioning CYSHCN.
